Maybe a blockage: A guy should be able to pee straight. If not, then his primary care doctor or a urologist can take a close look at the meatus (the hole where the pee comes out) to see if there is some blockage. If there is blockage, some scarring, some narrowing (stricture), or hypospadias (the hole is off-center), the pee might spray all over the place, or come out in a y, a v, a w, etc...
