Tough problem: A very difficult clinical scenario. Options are limited. Subscapularis tears don't heal on own. Pt, arthroscopic subscap repair, humeral head allograft transplant, resurfacing hemiarthroplasty are options but in setting of prior infection its risky. Consult a shoulder specialist for a full work up and discussion of options. A challenging case indeed. Good luck.

Not good: If you have had multiple surgeries and an infection and now have arthritis, it will continue to deteriorate over time and likely result in a shoulder replacement- which will help you if done correctly
