See a REI: The short answer is that very few women at age 46 are able to conceive under any circumstances, i'm sorry to say. If you and your gf seriously want a child you should seek help from a rei or fertility specialist as soon as you can to discuss options. Because of the significant concern about her age i would go directly to a specialist, not to a primary or regular ob, no matter how good they are.
