Healing site: I am confused if the extraction site actually reopened as you say or if it just feels different with your tongue. After extractions the site undergoes a natural shrinkage during the healing phase as the inflammation resolves. Next time you see your dentist mention your concerns to ease your mind.

Sinus affected..: Could be when your upper molar was pulled that you sinus was perforated. Fairly common. Those do take awhile heal and may need the help of a dentist. It could have reopened. Best to go see the dentist or surgeon who did the extraction rather than have a phone diagnosis.

Oral/antral fistula: Sounds like an oral/antral fistula. This is a communication between your sinus and your mouth. See an oral surgeon about this. Can be very hard to close and keep closed, but these days there are great things that can be done. See the surgeon.
