No: A dowager's hump will not affect carotid artery flow. Disease in the carotid artery will affect flow.

Artery Ds is Key: Artery disease, typically starting in childhood, yet asymptomatic for decades, is primary driver of decreased artery flow. The primary correlates/drivers of artery disease: age, smoking, lipoproteins (not cholesterol), blood sugar (well below what is called diabetes mellitus), blood pressure & multiple other factors. In that dowagers hump is correlated with age, will also correlate with artery ds.
