Combined: Treatments involve surgical lumpectomy to remove the lesion, possible lymph node removal depending on size & stage if the lesion, possible chemotherapy or hormone therapy, possible radiation therapy. Need a good medical oncologist to help coordinate the best treatments.

Depends: Several factors are important: grade of the tumor, estrogen receptor status, her2 neu status. If er+, her2 -, grade 1, hormone therapy alone. If er-, her2-, then would possibly require chemotherapy. If er +, her2+, may require chemotherapy, Herceptin (trastuzumab) and hormone therapy. Also would depend on what surgery is performed. This requires a sit down discussion. Hope this helps.
