Many reasons: Tia or stroke, low blood sugar, very high blood sugar, inappropriate use of medications, infections, alcohol, low sodium, this list is incomplete.

Several things: It could be a symptoms of low blood sugar after too much diabetic medication, or it could be just a symptom of alzheimer's disease. It could be also a brain infarct and the condition should be investigated by a physician.
