How does a dermatologist shrink large pores?

Lasers or peels: Many ways to shrink large pores including many different laser devices (ie: fractional co2, activefx, deepfx, totalfx, re:pair, re:store, erbium:yag, fraxel, pixel, thermage). Chemical peels such as trichloroacetic acid (tca), glycolic acid, jessner's peel (lactic acid, salicylic acid, resorcinol). Microdermabrasion also helps.

Difficult: Pore reduction is difficult...Some advocate chemical peels, laser resurfacing, and / or the use of a topical retinoid.
