Needs surgery: Once symptomatic, no real alternative other than surgery. If you live near a medical center, look into a " resident clinic". If you are in a small town, work with your local hostipal to set up a deal - frequently steeply discounted ( be prepared to show your income statement, tax return, etc.).

Surgery: Not much option for any "treatment" that i know of short of surgery. Try Ibuprofen or something similar, but that will not stop the pains. See if there is any medical support from a local community service, or contact a local hospital if they know of community support services. Otherwise go to an emergency room with th pain where they are obligated to treat you, but there would still be a bill.
