Not always: Many doctors now perform this under local anesthesia in the office. It can be done with a needle through your neck, or through your mouth. Doing it through the mouth while you're awake can be pretty tricky. The problem can be reimbursement from insurance when done in the office. Especially getting the Botox paid for.
