Tonsils ascend: The cerebellar tonsils can ascend as you age. I would consider MRI cervical spine to make sure there is no syrinx (hollow cavity within the cord).

ACM?: Not sure of that one but pain without evidence of tissue damage may need a top down approach. It could be exacerbated by anxiety. Do you sleep well? Do you clinch your teeth at night, ? Are you treated for mood or stress. Drugs like Cymbalta (duloxetine) at 30 mg help anxiety and peripheral pain. If not sleeping try .5-1 mg Clonazepam at night for 2 weeks then as needed for sleep, help if discovery made.
