Why do drs not feel it is important to medicate a person with high cd4 counts?

Various opinions: There are many doctors who treat HIV disease who will begin medications even when the CD4 cell counts are high. This will depend on a variety of factors, and the actual levels. The philosophy of withholding treatment until levels are low has been disproven and is in fact harmful.

Treat everyone: By medicate, are you referring to treatment of HIV infection with antiretroviral medications? Current U.S. Treatment guidelines recommend treatment for everyone with hiv, regardless of CD4 count. Doctors who don't know that probably aren't HIV experts.
