Ultrsonic cleaner: Ultrasonic cleaners are very effective especially when used in conjunction with antimicrobial mouthwash.

Elbow grease: The most effective way is good old fashioned elbow grease. Scrub it with a toothbrush and tooth paste. There are several types of fizzy tablets on the market which are also helpful. Try to avoid things with alcohol (mouthwash) as this can degrade some types of plastic.
