Many reasons: Women suffer from incontinence more than men. This is usually due to stretching of the pelvic floor with labor and damage from delivery or episiotomies. Over time, the muscles become weaker, especially if they have severe constipation. This can lead to prolapse (turning the rectum inside-out) and further damage to the sphincter muscles. Many people have diverticulosis also.

Depends: It can be caused by many different reasons from disorders of the nerves, damage to the pelvic support, digestive issues leading to soft stool/diarrhea.....Recommend follow up with a gastroenterologist or colorectal surgeon to evaluate cause and treatment.
