Tricky question: You should be able to contact either the surgery center or hospital where the surgery is scheduled for an accurate estimate of costs. But your costs may vary significantly depending on your insurance coverage, deductible etc. If you are paying cash, the formula should be straight forward.

Agree cost hand: Agree contact the surgery center or hospital in general physican fees are the same at either but asc;s ambulatory surgery centers that are not owned or billed for by hospitals cost less in the us. ( their lobbyists are not as strong as the hospitals are) so if u you have a high deductible or co-pay that includes the facility fee, ask what those costs are you can save money ).
