Has any musician/ player died from creutzfeldt-jakob disease?

Possibly: This is a very rare prion related disease that affects very few people. The profession of the patient is probably not related, unless you are a butcher or a slaughter-house worker -- which seems unlikely for a musician -- but it is possible.

None I know of: Creutzfeldt-jacob is by definition not due to occupational exposure (as in british slaughterhouse workers), though the disease mechanism and basic pathology is the same. Cjd is prion disease without any known exposure. I know of no celebrity musicians who died of this thankfully-rare disease which takes maybe 200 americans per year; being a musician is no risk.
