Yes: If incompletely removed, a fibroadenoma can grow back. Alternatively, it is possible to develop a second, unrelated fibroadenoma in the same breast.

Unlikely: If completely removed it should be gone. Scar tissue in the area can be confusing. Another fibroadenoma can occur in that or other breast.
