Depends on area.: Third degree burns leave an eschar, which is total death of the skin, dermis down to the underlying subcutaneous fatty layer. A small area of 3rd degree burn can be treated locally, requiring subsequent surgical debridement of the eschar. Larger area of 3rd degree burn requires surgical debridement on a larger scale, hence surgery.
