Bleeding . . .: A positive pregnancy test with bleeding, particularly heavy bleeding with cramps, is most suggestive. If pregnancy symptoms like fatigue, urinary frequency, breast tenderness, nausea, and vomiting suddenly improve, or if none of those symptoms appear despite the positive pregnancy test, you should be suspicious.

Several: Signs of an early miscarriage include your symptoms of pregnancy, like breast tenderness, fatigue, nausea, etc., going away. The more obvious signs are bleeding. There are some women that have little to no symptoms of pregnancy and everything is fine. Also, up to 23% of women bleed in early pregnancy, and again, everything can be fine. An ultrasound should be able to let you know things are ok.
