Can chronic sinus problems cause extreme hoarseness?

No: Although chronic sinusitis can cause many symptoms in the airway, severe hoarseness is not usually one of them. When I have patients with severe hoarseness I am concerned about reflux and about vocal abuse and poor vocal technique far more than i'm concerned about sinus problems.
