Rare: Most of the time enlarged tonsils are due to infection, mostly in children, in adults rarely malignancy can occur, bilateral in lymphomas, unilateral enlargement in cancer, and needs professional evaluation.

Rarely: Cancer in a tonsil is usually on one side or the other and is very clearly abnormal. If yours have been big for a while, don't be concerned about this. If they obstruct your breathing, especially at night, it might be wise to have them removed.
