Could be: In-utero neurotrauma/toxicity (oxygen/blood loss, or alcohol/mj/nicotine/drug/virus exposure) short gestation (<39 weeks), birth trauma, concussion, severe head injury, chemotherapy/radiation exposure, infection, poisons (lead, etc), poor nutrition can all worsen inherited adhd or cause a condition similar to adhd.

Hard to say: Although diagnostic criteria state that symptoms of adhd can not be better explained by another diagnosis, neurological damage could due to numerous factors could cause significant cxhanges in slow processing, hyperkinesis, innatention to merit a diagnosis of a dhd.

It Could Be: Typically, we see more of a functional change related to ADHD. However, it is not uncommon to see shifts in ability to concentrate in those who have suffered Traumatic Brain Injury, like boxer's and Chronic Traumatic Encephalitis.
