Likely from birth: The cause of keratoconus is not fully understood. We do know that there are both genetic and environmental factors. People with this condition have a defect in the structural proteins which give the cornea strength. Some research indicates that allergies and eye rubbing also play a role.

Keratooconus: Keratoconus appears to be related to excessive eye rubbing. While keratoconus can often be seen in families (suggesting a genetic component), other patients have no family history. Severe eye rubbing is a common finding, leading to weakening of the cornea.
