Neuromas: Only if conservative treatment fails. These treatments may include: cortisone injections, orthotics, therapeutic neurolysis via dilute alcohol injections (my favorite) and more.

Not necessarily: Injection therapy is very successful, orthotic therapy is also very helpful when a biomechanical issue can be found to be a cofactor in causing the neuroma to develop. Conservative therapy should be tried first.

Sometimes.: Conservative treatmetns are used first, such as cortisone injections or sclerosing alcohol injections. Wear shoes with a stiff sole and soft uppers that are also wide enough for your foot. When teh conservative measures fail, surgery is usually successful.
