Need proper exam: Being in 'great shape' doesn't preclude the possibility of disease, injury, or dysfunctional vertebrae, or locked si joint. It could also be an organ problem, herniated disc, muscle imbalance, stretched ligament, quadratus lumborum spasm, etc., etc. A good physical exam, including a good osteopathic hands-on diagnostic exam might elucidate the problem.

Several reasons: It could be that it is genetic and back issues run in your family. There are spine abnormalities that can cause pain such as a spondylolisthesis. You may have a disc problem inspite if being in good shape or it could be referred pain to your back from another issue.
