Infection or cancer: Tuberculosis, a pneumonia or a cancer can present this way. There's no sense worrying about it. Just walk it through with a doctor. It will require further imaging, skin testing in specific people, maybe a biopsy. This is no time to think or philosophize. Just get it done. If it's TB or another infection you will want treatment quickly. You will want answers quickly in any case. Best of luck.

Could be several...: But what you want to rule out first is cancer. Until proven otherwise, a lung nodule with associated mediastinal lymph node enlargement is worrisome for cancer. The two could be two unrelated benign conditions (ex. Granuloma and inflammatory changes in the mediastinal nodes). A pet scan could help to further evaluate it.

Benign or malignant: Multiple possibilities. It's critical to obtain a thorough evaluation. All patients found to have a lung nodule should be referred to a comprehensive multidisciplinary thoracic surgical oncology program. After review of characteristics of nodule, patient history, risk factors, and examination; an individualized plan can be devised to facilitate diagnosis and/or treatment.
