Shorten achilles: With constant use of wearing high heels above 3 inches, may be shortening of the achilles tendon. Further with a very high heel, this also puts a lot of pressure on the front of the foot that may cause pain to and around the joints, including the big toe joint.

Several things: They can cause a functional shortening of the achilles tendon, which will lead to pain when you wear flats. They can cause pain of the front of the foot or can create a neuroma.
