Caution: I think conservative treatment should be exhausted before contemplating surgical treatment. If all else fails then surgery really becomes the option.

Can be: Make sure that prior to surgery to either of theses conditions you have tried all conservative care. Both of these conditions are usually succesful without surgery. If all else fails, then surgery would be a good option.
