More than nursing: Stage 4 pressure sores extend to muscle or bone and generally are past the point where they will heal with pressure avoidance or nursing measures--they usually need surgery. The problem, though, is not cured with an operation--it can easily recur. So, before operating, it is necessary to know the risk of recurrence. If it is very high, it is better not to try to definitively treat the sore.
