No: Although coughing up blood (hemoptysis) can occur at any time with lung cancer it is usually not the leading symptom. Most people lose significant amounts of weight and may pass away from infection, sometimes they become increasingly short of breath or they may just fall asleep and not wake up. The way they pass is not important it is how they pass. It should be relatively pain and anxiety free.

Sometimes: Spitting out blood happens when the cancer opens up into an airway. While not necessarily the symptom before death, it usually signifies pretty advanced disease. Patients can also suffocate on the blood if there is a lot and it is not controlled in a timely manner.
