Many possibiities: Anyone experiencing severe flank pain needs a full evaluation including imaging and blood work and urine analysis - while musculoskeletal pain is possible, other possibilities include kidney stones, pyelonephritis, appendicitis, visceral perforation, liver abcess, ascending cholangiitis etc etc.
