New oil glands: At about 3-4 weeks, babies develop their oil glands, and for the next few months, some of them are prone to acne like teenagers. It's normal. It's usually gone by the first birthday, or you can ask your pediatrician whether a topical agent might be appropriate.

Transient vs bad: Babies float in moisture during pregnancy and have pristine moisturized skin at birth. By 2 wks many show the effects of retained maternal hormones and develop transitional baby acne (2wk-2.5m). For most it looks bad but is superficial and fades without treatment. There are a few that will have disordered hormonal effects and need treatment.Any acne after 3mo deserves a special evaluation
