What is the most effective way to relieve swelling from poison ivy?

Prednisone: Nothing beats systemic corticosteroids for poison ivy, poison oak or poison sumac. Or any contact dermatitis for that matter.

Steroids: If the area is large systemic steroids - prednisone- is great. If area is small mid to high potency topical steroids may be good choice.
