Not Always: But if he sees something that he can improve upon, like fraying a very sharp point, that's acceptable. Sometimes we see area's of teeth, called plunger cusps that can elimiated a problem smoothing down the protruding area, similar to smoothing off a rough spot.

It helps: The xray helps dentists determine the depth /size of the decay, and therefore the proximity to the nerve. If the decay, on xray, clearly shows the nerve is involved, then you may have more than just a simple filling.

X-ray?: I assume "fray" is a typo. X-rays are used to see tooth and bone structures not visible to the naked eye. A full set of diagnostic dental x-rays is less radiation than you would receive on a 1 day ski trip. Dentists take only the x-rays necessary to allow them to do a complete diagnosis of your problem.
