How can an ophthalmologist detect signs that lasik had been performed on the eye?

Observation: The ophthalmologist will look at your cornea. The lasik flap, flattens down nicely but the edges of the flap are easily seen at the very high magnification power of the instrument used to observe the cornea (the slit lamp). It is unmistakeable.
