Surgical Rx for IBD: Ileo anal reservoir is a surgical rx for patients with ulcerative colitis -uc (typically). Uc pts with long term and/or extensive disease have increased risk for colon cancer. Such pts with precancerous changes get surgical removal of colon and rectum. The ileum (part of the small bowel) is attached to the anus, often with a surgically constructed pouch to act as a fecal reservoir.

J-pouch: It is a way of removing the entire colon without the need for a permanent colostomy. The end of the small intestine (ileum) is sewn in such a way to make a j-shape which allows for more reserve for stool to allow for fecal continence.
