Depends.: "misuse of back" is a common problem, and physical therapy can help with that.

Needs evaluation: Might be rheumatological problem. If this has been ruled out would suggest pt, antiinflamms, possible cortisone injections. If still a problem needs evaluation by bone scan , ct scan and/or MRI for further information. Sometimes is mechanical.
