Depends: If it is not giving you a problem other than the look, i would not do anything. If it is causing problems then it can be evaluted and the toe removed.

If there is pain: There is surgery that can be done to deal with the problem. If there is no discomfort, then it is largely a cosmetic issue.

6TOES ON FOOT: If you are conscious of it and it looks ugly, have it excised the one which looks ugly and is placed wrong.We can only advice you by looking at it or a photo. But if you have no concern with how your foot looks, than you can live with it. So you decide and talk to your doctor or podiatrist.
