High: By survival rate i assume you mean how long the implant will work and not how long long you will live after surgery. Most patients undergoing total hip arthroplasty survive their surgeries and go on to enjoy their new hips. The hips themselves can last 20 years or more now.

High: If you mean the rate of survival for the patient, overall it is over 99.9%, especially for someone of your age. If you mean the survival of the implant over time, expect a modern total hip that is implanted correctly to last over 30 years.
