Sometimes: A tracheostomy is usually performed to allow long term ventilation by a mechanical ventilator. If COPD is so severe that a ventilator is required, a tracheostomy may be performed. It is not a treatment for COPD otherwise.

Not really: While patients with COPD may get a tracheostomy in order to get off a ventilator, it is not standard therapy for nonintubated patients.
