See your surgeon : There may be residual cyst or foreign body such as hair strand underneath that cause periodic inflammation and may need to re-excise it.

Surgery: The description you are giving is typical of pilonidal disease. The only treatment for pilonidal disease is surgery. There are numerous surgeries offered for this. Ultimately, it will be what your surgeon is most comfortable with and has done most often.
