Not most common: The internal mammary artery is the best graft for coronary artery bypass grafting (cabg), saphenous vein used for backup. One of the best operations ever invented, the left internal mammary artery to coronary artery bypass graft is the gold standard by which almost any vascular operation is compared to with a patency of over 90% at 10 years. There is almost no operation with a patency that high.

Not sure: what you're asking. it's used quite commonly. it is harvested using an endoscopic or open technique. usually, one end is connected to the aorta, the other end is connected to the coronary artery past the blockage.

CAB and Saphenous: The saphenous vein is the usual vein used for coronary artery bypass for several reasons. It is easily accessible under the skin. It is long and usually of adequate diameter. It is technically easy to remove and thee are saphenous veins in both legs. Also, removing the saphenous vein does not cause any untoward effects.
