Yes: Painful spleens as can occur with autoimmune diseases, lymphomas, viral illnesses (mononucleosis), and trauma can cause pain. The typical symptom is pain in the left upper abdomen that may also be referred to the left shoulder. This is particularly significant in the setting of a ruptured spleen. Enlarged spleens can hurt with direct examination.

Spleen : If a spleen is enlarging relatively quickly from bleeding or infection, pain can occur from stretching the capsule.
