No: Heart palpitations is a symptom. Causes of death are as a general rule not symptoms. For example, someone could die from a heart attack and for some people (especially women) there may be no symptoms except loss of appetite (i.e. Loss of hunger).

Very unlikely: This is rather unlikely, but can potentially happen, dependent on what the palpitations represent. If the palpitations are the result of a malignant cardiac rhythm, then they can lead to death if left untreated. Also, if they lead to fainting spells, they may lead to accidental deaths (fainting while driving or being in a dangerous situation).
