See below: The best way to do this is to discuss your questions with the rehab team. They are experienced in dealing with the vast majority of issues related to home discharge. The case manager often takes the lead on this and the rehab doctor also is usually very experienced in this issue.

They will provide education and training for not only the patient but the family/caregivers, recommend specific equipment, and assist with the transition to an appropriate home rehab or outpatient therapy program. It is also important that a follow up appointment is scheduled with the family physician and all specialists involved with care.
