Working closely with: Working closely with the rehab team is crucial. They will provide education and training for not only the patient but the family/caregivers, recommend specific equipment, and assist with the transition to an appropriate home rehab or outpatient therapy program. It is also important that a follow up appointment is scheduled with the family physician and all specialists involved with care.
