Yes: There is no test which if normal guarantees that you will not get a heart attack. And the echocardiogram is not good for predictiong a heart attack anyway.

Yes it is possible: A normal echocardiogram only suggests that the heart function is good at the time of the test. It does not assess the risk of heart attack. If you however combine a stress test and echo (stress echo) it can assess the risk of having a heart attack. A normal stress echo makes the risk of heart attack over the next year low but not zero. Heart attacks can still happen by a sudden blockage.
