Economics.: It doesn't pay. Almost no one but the patient benefits from health... Almost everyone but the patient benefits from sickness. Follow those dollars. Things may change, but these wheels turn very slowly. Preventive care may actually end up costing society a great deal as people live longer - more billable ailments and increasing complexity of care with longer period to collect earned benefits.
