With a normal testosterone i would guess the hirsutism is constitutional. Painful menses needs evaluation to exclude endometriosis, fibroids, ovarian cysts, adenomyosis. Smoking forbids bcps ?Try motrin/ aleve (naproxen).Check thyroid.
