Zyrtec (cetirizine) Cat B: Zyrtec has been shown to pass through breast milk in animal studies, and is believed to also pass through human milk. Zyrtec has the potential to cause excess sedation and irritability in young infants, and can interfere with normal lactation. Alternative medications such as decongestants are also suggested to be used with caution. Nasal saline spray is the safest, but admittedly not as dramatic

A few options: The OTC Rhinocort of Nasonex (mometasone) are often a better choice since they never enter your milk and do have an anti-inflammatory effect. Nasal saline washes are also a help. Non-sedating daily antihistamines like loritadine or fexofenadine have larger molecules and do not transfer in significant amounts into your milk.
