Usually yes: Risser sign of 4 means the skeleton is virtually mature. Surgery for scoliosis at this stage does not result in much improvement in the spine curvature, and usually requires neurological symptoms as well before deciding to operate. I understand that Scheuermann's disease is not commonly treated with surgery. Hope that helps.
